**Please Read Before Setting Your Appointment**

If your are setting your appointment for Live Scan Service, please make sure to bring the following:

• Fully filled out Request for Live Scan Service Form
• Valid Photo ID (ex. CA Driver’s License, Passport, Identification card etc.)

We will not be able to process your Live Scan if you do not have either of these items. ID is also necessary for Ink Card and other services.

Cashiers Checks/Money Orders

(We do not accept personal checks)

Please bring your completed REQUEST FOR LIVE SCAN SERVICE form, one form and your payment.
